Service Coordinator - Job Description
TITLE: Service Coordinator, Assessment Services
PURPOSE OF POSITION:
Responsible for providing effective vocational assessment, skills training, and service coordination to participants involved in transition to community employment.
ESSENTIAL REQUIREMENTS:
Bachelor’s Degree in Human Services or related fields such as Psychology, Sociology, Education, or Rehabilitation. Two years experience preferred.
Ability to effectively communicate both orally and in writing with participants, referral sources and employers on an individual basis as well as in a group setting. Relates to participants, referral sources, and employers in a respectful and professional manner.
Ability to coordinate multiple tasks, process and prioritize information, and create and implement plans of action.
Works independently and uses good judgment. Must have good problem solving skills and a creative approach to training and service coordination.
Ability to perform crisis intervention and First Aid/CPR. Full body range of motion and physical ability to lift up to 30 lbs.
Flexibility--must be able to work a variety of hours, including weekends when necessary.
Valid driver’s license, proof of current automobile insurance and a reliable car.
Must receive an acceptable background check including police check.
ESSENTIAL DUTIES:
Develop, maintain, and implement individual assessment, rehabilitation, behavior and training plans with participants and submit necessary reports. This includes the administration, scoring and interpretation of testing.
Facilitate pre-admission and referral and communicate program plans and progress to relevant staff throughout program and during transition to other work services. Develop and implement appropriate modifications. Maintain follow along programs including all meetings and paperwork for individuals in follow along status.
Serve as a significant role model and resource for effective and appropriate work behaviors, procedures and practices.
Establish and maintain good working relationships with Easter Seals WRC, referral sources, community employers and community-based training sites.
Conduct job analysis, task analysis, and time studies as needed.
Communicate necessary information to other Easter Seals WRC staff, referral sources and other team members through regular communication, staffings and reports.
Provide crisis intervention and CPR/First Aid as needed.
Perform other duties as required by program and/or organization needs
